Having read and understood the Media Lab policies and procedures I, the undersigned, willingly agree to abide and follow these policies and procedures. I willingly accept the financial responsibility of repairing or replacing any Media Lab equipment that is damaged, lost, or stolen while the equipment is checked out by me or my class work group.
